Survival of a fetus after advanced abdominal pregnancy-challenging obstetric concern: a case report

Abdominal pregnancy is a very rare form of ectopic with high morbidity and mortality for both mother fetus. A 29 years old Bangladeshi woman presented from rural area to Chittagong medical college hospital first gynecological evaluation after 36 weeks amenorrhea lower abdominal pain. An ultrasound revealed live fetus sub amniotic collection possibility ruptured uterus. Laparotomy was done weighing 2500 gm delivered. Placenta adhered GIT bladder. kept in situ injection MTX given postoperatively. After re-laparotomy postoperative pelvic abscess hemoperitoneum, placental tissue removed hemostasis maintained. Both are discharged good health. can be delivered an advanced pregnancy. Whether the placenta should or removed, it controversial. literature review, we propose management this regard. Advanced extremely diagnosis requires index suspicion. The life-threatening complication bleeding detached site. avoid intraoperative bleeding. involution during follow-up by ultrasound, color Doppler β-HCG serum level decrease.
